By submitting this request, I understand and agree that New Mountain Top Baptist Church — including its Pastors, Elders, Officers, Agents, Employees, and Members — is not responsible for any injury, illness, death, or damage to personal property that may occur while my request is being reviewed, approved, or denied.

I release the church and its representatives from any related claims or liability. I also agree to protect and indemnify the church and its representatives from any financial loss connected to my request or actions during this process.
